Parish Census

The Parish Census is our attempt to ensure that we have the correct information in our new parish database for all registered parish households.  Census forms populated with our current information for Parish households were distributed at church and then, mailed to all registered households.  Our goal is to update every parish household record.

   •  Download the Registration and Ministry Matrix Form or pick-up the
       “Registered Parishioners’” Packet from the racks at the church exits
        or from the Parish Office.
   •  Complete the forms.
   •  Return the form by mail to:

                 Census Form
                 1201 E Anderson Drive
                 Palatine, IL 60074
   •  Drop the form at the Parish Office or at the drop boxes at the church exits
   •  Fax the form to the Parish Office at 847-934-4919
