The Parish Census, 2009, is our attempt to ensure that we have the correct information in our new parish database for all registered parish households.  Census forms populated with our current information for Parish households were distributed at church and then, mailed to all registered households to be received around March 1, 2009.  To date, over one thousand forms have been returned.  Our goal is to update every parish household record.

 If you HAVE your populated census form, please:
   •  Read it.
   •  Make changes so that your form reflects your current household situation.
   •  Add e-mail addresses.
   •  Complete Ministry Matrix on Side Two.
   •  Return your form in the provided envelope as soon as possible.

 If you have LOST or DON’T HAVE your populated census form, please:
   •  Download the Registration and Ministry Matrix Form or pick-up the
       “Registered Parishioners’” Packet from the racks at the church exits
        or from the Parish Office.
   •  Complete the forms.
   •  Return the form by mail to:

   •  Drop the form at the Parish Office or at the drop boxes at the church exits
   •  Fax the form to the Parish Office at 847-934-4919
